The fact sheet form is the rotation tracking method used by the AHEC office to gather the necessary information to ensure your rotation needs are properly met and to ensure any reimbursements due to you are correct. The AHEC Fact Sheet also provides the program with reliable data on student and resident rotations, and it serves as a mechanism for communicating among AHECs statewide to activate specific services for students and residents as they initiate their rotations. If for any reason a rotation is canceled or a site changed, please contact the AHEC Central Office immediately.
Students must complete an AHEC Fact Sheet as soon as rotation information is confirmed; this fact sheet must then be returned to the AHEC office. The fact sheet can be obtained through the college or department’s AHEC Coordinator, the AHEC Central Office or electronically from the AHEC website.
Every UK student/resident who takes an off-site rotation is expected to complete this form prior to the rotation.
